Changing your PCM and actually getting a referral.
TRICARE Prime requires you to have a Primary Care Manager (PCM) and to route most specialist care through them. The mechanics are not on the back of your card. This page walks the actual flow: how to change your PCM via your regional contractor (Humana for East, TriWest for West as of Jan 2025, International SOS overseas), how a referral request moves through the MTF Right of First Refusal, what to do when it stalls, and the escalation ladder when the system fails you.
Change your PCM — 6 steps
- 1Confirm you're on TRICARE PrimeOnly Prime (and Prime Remote) requires a PCM. TRICARE Select, US Family Health Plan, and Reserve Select either have no PCM or operate as PPO-style plans where any TRICARE-authorized provider is fine. If you're on Select and want lower co-pays via a primary provider, you're looking at choosing a network doctor, not formally "changing PCM."
- 2Log into milConnect → DEERSmilconnect.dmdc.osd.mil. CAC, DS Logon, or MyAccess for sponsors and dependents. Verify your DEERS enrollment shows the correct address and the family members you want to update.Watch out: PCM change requires you to be enrolled at the correct address. If you recently PCS'd and DEERS shows the old ZIP, update DEERS first — otherwise the PCM options will show the wrong region.
- 3Identify your region's contractorTRICARE East = Humana Military / HNFS, contracted as of 2018. TRICARE West = TriWest Healthcare Alliance (as of January 2025, taking over from Health Net Federal Services). TRICARE Overseas = International SOS. Each contractor has its own portal — you typically can't change PCM via milConnect alone; you go through the regional portal.Watch out: TRICARE West transitioned from HNFS to TriWest on Jan 1, 2025 — this was a major contract switch. If you've been told "log into HNFS to change PCM," that guidance is stale for West-region beneficiaries.
- 4Use the regional contractor's PCM-change portalEast: tricare-east.com (Humana) → "Find or Change My Doctor." West: tricare-west.com (TriWest) → "Choose a Doctor" or call 888-TRIWEST. Overseas: ICE portal at TRICARE Overseas Program. The change-PCM workflow shows you available PCMs at your assigned MTF FIRST (military doctors), then in-network civilian providers.
- 5Submit the change; receive effective datePCM changes are typically effective the FIRST OF THE FOLLOWING MONTH if submitted before the 20th of the current month — otherwise the first of the month AFTER that. You'll get a confirmation email + an updated TRICARE wallet card you can print or save digitally.Watch out: You may still see your old PCM's name on appointment portals during the transition window. Use the EFFECTIVE DATE on your confirmation — appointments scheduled before that date count against the old PCM.
- 6Update your TRICARE Online (TOL) Patient Portaltricareonline.com — link to the new MTF if your new PCM is military, or to your civilian PCM's network portal if civilian. TOL is where you book appointments, refill prescriptions, view labs, secure-message your PCM.
Get a specialist referral — 6 steps
- 1Confirm you need a referralTRICARE Prime requires PCM referral for MOST specialist care. EXCEPTIONS: mental health (self-referral allowed, no PCM permission needed), OB/GYN (self-referral for routine well-woman care), and emergency care (always covered, no referral needed). Active-duty members on TRICARE Prime Remote follow the same rules with a Prime Remote PCM.
- 2Request the referral from your PCMSchedule a PCM visit (in-person or virtual) to discuss the specialist need. PCM determines medical necessity; if approved, the PCM enters a REFERRAL REQUEST in their electronic system. This is the documentation that triggers the rest of the process.Watch out: A "phone consultation" with a PCM nurse is NOT a referral. The PCM physician must enter a formal referral, which goes through the MTF's referral management center (RMC) for processing.
- 3RMC processes — Right of First Refusal (ROFR)The MTF Referral Management Center has the RIGHT OF FIRST REFUSAL: they get to decide whether to book your specialty appointment INSIDE the MTF first. If the MTF can see you within TRICARE's access standards (typically 28 days for routine specialty), they'll book you in-house. If they can't, the referral converts to a NETWORK referral — you can see a civilian network specialist.Watch out: ROFR is where most beneficiaries get stuck for weeks. The MTF may "claim" the referral and then fail to actually schedule you. Track the referral status via TOL Patient Portal; if 5+ days pass without scheduling, call the RMC directly.
- 4Receive your Referral Authorization LetterOnce approved (in-MTF or network), you get a written Referral Authorization Letter via TOL Patient Portal or USPS. It lists the specialty, the authorized number of visits, the date range, and (for network referrals) the approved provider/practice.Watch out: Verify the AUTHORIZATION END DATE before booking. Specialty referrals typically run 6 months; if you miss it, you re-start the request process.
- 5Book the appointmentIn-MTF: through TOL or the specialty clinic's scheduling desk. Network: contact the authorized provider directly; they need your TRICARE ID and the referral authorization number. The provider verifies authorization with the regional contractor before the appointment.
- 6When the system stalls — escalationIf your referral has been in "submitted" status for 10+ business days without scheduling: (1) call the MTF Referral Management Center; (2) contact your TRICARE Beneficiary Counselor and Assistance Coordinator (BCAC) at the MTF; (3) call the regional contractor (HNFS for East, TriWest for West); (4) file a Beneficiary Complaint with TRICARE; (5) for active duty, your unit medical NCO / commander can press the MTF for action; (6) Congressional inquiry as a last resort.

Can I have a civilian PCM if I'm TRICARE Prime?
Yes — if you live in an area with limited MTF availability or specifically requested it. The TRICARE Prime "remote" enrollment automatically assigns a civilian PCM. Even at standard Prime, if no military PCM has open slots, you can request a network civilian PCM. The trade-off: military PCMs at MTFs are typically more familiar with military-specific things (deployment health, PHA processes); civilian PCMs aren't.
My PCM is terrible. Can I switch?
Yes. You can change PCM as often as you reasonably need to — there is no annual lockout. The mechanism is the regional contractor's portal (East: Humana; West: TriWest). Service-member preference matters: you don't need a clinical reason to request a different PCM. That said, switching PCMs too frequently can make your medical record continuity worse, which can backfire if you're managing a chronic condition.
What about mental health — do I need a PCM referral?
NO. This is the single most important TRICARE rule to internalize. You can SELF-REFER to mental health care under TRICARE. You do not need PCM permission, commander notification, or anyone else's blessing. Walk into the MTF behavioral health clinic, call Military OneSource (800-342-9647) for 12 free non-medical counseling sessions, or schedule a network mental health provider directly. There are MILITARY ONE SOURCE NON-MEDICAL counseling sessions that don't become part of your medical record at all.
What's urgent care vs ER under TRICARE Prime?
For Active Duty Prime: urgent care can be used WITHOUT a referral 4 times per fiscal year at any TRICARE-authorized urgent care center; beyond 4 you need a referral. For dependents on Prime: urgent care has no visit cap and no referral needed at TRICARE-authorized centers. ER visits never need a referral — but for non-emergencies, expect a higher cost-share if the visit isn't medically necessary. After-hours nurse advice line (800-TRICARE) before deciding ER vs urgent care vs wait-til-morning.
I PCS'd. How long do I have to update my PCM?
You should update your DEERS address within 30 days of arrival. PCM update is technically required within 90 days, but in practice you should do it ASAP — your old PCM is in a different region and you won't get appointments out there. Sponsor responsibility for the family's TRICARE enrollment falls on the active-duty member. PCS-related TRICARE issues are the #1 complaint to the TRICARE Beneficiary Counselor; don't wait.
TriWest took over the West region. What changed for me?
As of Jan 1, 2025, TriWest Healthcare Alliance replaced Health Net Federal Services (HNFS) as the West-region contractor. The TRICARE benefit itself didn't change; what changed is the PORTAL, the customer service phone number (now 888-TRIWEST), the network of contracted providers (mostly the same providers but the contract administration moved), and the claims processing pipeline. If you're West region and last used your HNFS portal login in 2024, you need to register fresh with TriWest at tricare-west.com.
How do I find an in-network specialist?
Use your regional contractor's provider directory. East: tricare-east.com → "Find a Provider." West: tricare-west.com → "Find a Provider." The directory lets you filter by specialty, ZIP, and accepting-new-patients status. Always call to confirm the provider is still in-network and accepting your specific TRICARE plan BEFORE booking — provider network status can change without notice.
